At this time, Forward Counseling & Consultation (FCC) is accepting Quartz, Group Health Cooperative of South Central Wisconsin, Dean Health Plan, United Healthcare / Optum, The Alliance, Medicaid, Dane County Comprehensive Community Services (CCS), Rock / Jefferson / Walworth County Comprehensive Community Services (CCS), Health Savings Account (HSA), and Self-Pay clients. Appointment time availability varies and will be scheduled on an individual basis with a 15-minute complimentary appointment if requested, please email or call for rates.
